You were fine during the day.
Then you lay down to sleep and suddenly your tooth is throbbing like something is trying to get out of it.
You are not imagining it. Tooth pain genuinely does get worse at night and there is a real reason why.
More importantly there are things you can do right now to get through the night, and things you need to do tomorrow morning before this gets serious.
When you lie down, blood flows more easily to your head. That extra blood flow increases pressure around an already inflamed tooth and makes the pain feel much more intense than it did when you were sitting or standing during the day.
On top of that your mouth produces less saliva while you sleep. Saliva is your mouth’s natural defence against bacteria. When it drops, bacteria become more active and an already irritated tooth gets more aggravated through the night.
There is also a simple distraction factor. During the day your mind is busy with work, family, and everything else. At night when everything is quiet, that tooth pain has your full attention and nothing to compete with.
There are several common causes and some are more urgent than others.
This is the most common cause of nighttime tooth pain in Cameroon.
A cavity that starts small and is left untreated keeps growing deeper into the tooth until it reaches the nerve.
Once bacteria get close to the nerve the pain becomes severe, especially at night.
If sweet food, cold water, or pressure when biting makes it worse, this is likely what you are dealing with.
This is the one you should take most seriously. An abscess is a pocket of infection inside or around the tooth.
The pain is usually a deep throbbing that does not stop. In Cameroon’s humid climate infections spread faster than many people expect.
If your pain comes with swelling in your face or jaw, a bad taste in your mouth, or fever, you are dealing with an infection and you need to see a dentist the next morning without delay.
Inflamed gums swell more at night and can cause significant pain around multiple teeth at once.
If your gums bleed when you brush and your pain feels more like a dull ache across several teeth rather than a sharp pain in one spot, gum disease may be the cause.
Many people grind their teeth while they sleep without knowing it.
The constant pressure wears down the enamel and strains the jaw muscles, causing pain that feels like it is coming from the teeth but is actually from the surrounding tissue.
If you wake up with jaw soreness or headaches alongside tooth pain, grinding is likely involved.
During rainy season in Cameroon many people experience sinus congestion from colds or allergies.
The roots of your upper back teeth sit very close to your sinus cavity.
When your sinuses are inflamed they press against those roots and create pain that feels exactly like a toothache.
If your pain is only in your upper teeth and you also have a blocked nose or cold, this could be the explanation.
These will not fix the problem but they will help you get through the night.
Mix one teaspoon of salt into a glass of warm water and rinse your mouth for 30 seconds.
Salt reduces inflammation and cleans bacteria from around the painful tooth.
Use what is already in your kitchen.
Do this two or three times.
Wrap some ice in a cloth and hold it against your cheek on the side where the pain is.
Keep it there for 15 minutes then remove it for 15 minutes.
This reduces swelling and numbs the area enough to take the edge off the pain.
If you have clove oil at home, dab a small amount onto a cotton ball and press it gently against the painful tooth.
Clove contains a natural pain reliever called eugenol that dentists have used for generations.
t will not last all night but it gives real temporary relief.
Take paracetamol such as Cipladon or ibuprofen from your nearest pharmacy following the dosage on the packaging.
Ibuprofen is slightly better for dental pain because it also reduces inflammation.
Do not place aspirin directly on your gum. It does not help and it burns the tissue.
Sleep with an extra pillow so your head is raised.
This reduces the blood pressure build up around your tooth and can noticeably reduce the intensity of the pain through the night.
Do not eat or drink anything hot, cold, or sweet before you sleep.
All three will make the pain significantly worse.
Do not smoke.
Do not press or poke the tooth with your tongue or finger.
Most tooth pain can wait until morning for a dentist appointment. But some situations cannot wait.
Go to a clinic or pharmacy tonight if you have any of these. Swelling in your face, jaw, or neck. Fever alongside the tooth pain. A bad or bitter taste in your mouth that will not go away. Difficulty swallowing or opening your mouth fully. Pain so severe that nothing is giving you any relief at all.
These are signs of a spreading infection. In Cameroon’s climate infections move fast. A dental abscess that feels manageable tonight can become a serious medical emergency within 24 to 48 hours if left untreated. Do not wait.

Call your dentist as soon as they open and describe your symptoms. Do not wait to see if it gets better on its own. It will not. Tooth pain at night is your body telling you that something is wrong and needs professional attention.
